چکیده

During the last decade, Gram-negative clinical isolates, including Enterobacteriaceae, Pseudomonas spp. and Acinetobacter spp. with decreased susceptibility to carbapenems, have been increasingly reported in Europe. Carbapenem resistance in Enterobacteriaceae may be related either to the association of a decrease in bacterial outer-membrane permeability with overexpression of b-lactamases possessing a very weak carbapenemase activity, or to a b-lactamase with strong hydrolytic activity towards carbapenems, i.e. a carbapenemase. Differentiation of these mechanisms may have consequences for the clinical management of patients and prevention of nosocomial outbreaks. – 5 The main carbapenemases described in Enterobacteriaceae are the Ambler class A KPC enzymes, Ambler class B b-lactamases (metallo-b-lactamases) of VIM, IMP and NDM types, and Ambler class D carbapenemases of theOXA-48type. Thecarbapenemase-producingEnterobacteriaceaeareusually resistanttomanyotherb-lactamandnon-b-lactam antibiotics, giving rise to multiresistant and pandrug-resistant isolates. Methods

Dissemination of carbapenemase-producing Enterobacteriaceae and Pseudomonas aeruginosa in Romania.

Fifteen carbapenemase-producing Enterobacteriaceae isolates and 12 carbapenemase-producing Pseudomonas aeruginosa isolates were recovered from patients hospitalized between August 2011 and March 2013 at the Hospital of Infectious Disease, Cluj-Napoca, Romania. One KPC-, nine NDM-1-, four OXA-48-, and one VIM-4-producing Enterobacteriaceae isolates along with 11 VIM-2-producing and one IMP-13-pr...

Covert dissemination of carbapenemase-producing Klebsiella pneumoniae (KPC) in a successfully controlled outbreak: long- and short-read whole-genome sequencing demonstrate multiple genetic modes of transmission.

Background Carbapenemase-producing Enterobacteriaceae (CPE), including KPC-producing Klebsiella pneumoniae (KPC-Kpn), are an increasing threat to patient safety. Objectives To use WGS to investigate the extent and complexity of carbapenemase gene dissemination in a controlled KPC outbreak. Materials and methods Enterobacteriaceae with reduced ertapenem susceptibility recovered from rectal s...

Epidemiology of Carbapenemase-Producing Enterobacteriaceae and Acinetobacter baumannii in Mediterranean Countries

The emergence and global spread of carbapenemase-producing Enterobacteriaceae and Acinetobacter baumannii are of great concern to health services worldwide. These β-lactamases hydrolyse almost all β-lactams, are plasmid-encoded, and are easily transferable among bacterial species. They are mostly of the KPC, VIM, IMP, NDM, and OXA-48 types. Their current extensive spread worldwide in Enterobact...

Predominance of KPC-3 in a survey for carbapenemase-producing Enterobacteriaceae in Portugal.

Among the 2,105 Enterobacteriaceae tested in a survey done in Portugal, 165 were nonsusceptible to carbapenems, from which 35 (26 Klebsiella pneumoniae, 3 Escherichia coli, 2 Enterobacter aerogenes, and 3 Enterobacter cloacae isolates and 1 Klebsiella oxytoca isolate) were confirmed to be carbapenemase producers by the presence of 30 Tn4401d-blaKPC-3, 4 intI3-blaGES-5, and one intI1-blaVIM-2 ge...

Molecular Characteristics of First IMP-4-Producing Enterobacter cloacae Sequence Type 74 and 194 in Korea

The worldwide dissemination of carbapenemase-producing Enterobacteriaceae (CPE) has become a major therapeutic concern in clinical settings. Enterobacter cloacae is a major pathogen that causes serious hospital-acquired infections. We investigated the clinical characteristics and molecular mechanisms of the first IMP-4-producing E. cloacae clinical isolates in Korea. Five carbapenemase-producin...

Extended-spectrum beta-lactamase- and carbapenemase-producing Enterobacteriaceae among Ethiopian children

BACKGROUND Infections by extended-spectrum beta-lactamase- (ESBL) and carbapenem-resistant Enterobacteriaceae (CRE) are an emerging problem in children nowadays. Hence, the aim of this study was to determine the prevalence of ESBL- and carbapenemase-producing Enterobacteriaceae among children suspected of septicemia and urinary tract infections (UTIs). METHODS A cross-sectional study was cond...
